For critically ill children from birth to age 18 and blood 25OHD less than 50 nmol/L
10000 IU/kg of cholecalciferol (max 400,000 IU)
Second dose if low Day 3 blood test
<50 nmol - 2nd loading dose of 10,000 IU/kg (max 400,000IU)
50-75 nmol -2nd loading dose of 50,00 IU/kg (max 400,000IU)
