Hip surgery with multiple doses of 50,000 IU of vitamin D weekly both before and after – RCT 2023

Active Comparator: High Vitamin D3 Supplementation

Subjects enrolled into Group 2 (high-dose Vitamin D3 supplementation) will receive 50,000 IU of oral Vitamin D3 (Cholecalciferol) twice per week for 1 weeks followed by 50,000 IU once per week for 3 weeks prior to surgery.

After surgery they will receive 50,000 IU of oral Vitamin D3 once per week for 4 weeks followed by 800 IU daily for 8 weeks. Vitamin D3 supplementation will be given for a total of 4 months.

This RCT will probably be successful.

Even better success predicted if they had

1. Use larger repletion dose: 250,000 IU is nice, but 400,000 IU is typical before surgery

   • Surgeries often deplete Vitamin D - 300,000 IU resulted in little response – Nov 2018

2. Repletion dose were proportional to BMI (dose either being more often of larger)

• Pre-operative Vitamin D often helps (thyroidectomy in this case) – June 2021

• Taking Vitamin D just before and after surgery helps (open-heart in this case) – RCT Feb 2021

• Total hip replacement: 14 times more likely to redo if low vitamin D – March 2017

• Better total hip replacement if have high vitamin D

• Death of older hip fracture patients at least 1.6X more likely if low vitamin D – May 2018

• Perhaps fewer bone non-unions with 100,000 IU weekly of vitamin D – RCT 2018

• Hip replacement 2X more likely if hip bone surgery was during winter (low vitamin D) – Dec 2014

• Hip fractures reduced 2X to 6X with just 10 minutes of sunlight daily – RCT 2003-2010

• Guideline following hip fracture – 50000 IU vitamin D daily for 7 days – Jan 2013

• Hospital found that Vitamin D saves money by improving bone fracture healing– March 2016

  • Unfortunately many hospitals realize that they get more business/income if they do NOT recommend vitamin D

• Surgical Site Infection 4X more likely if low vitamin D – Aug 2019
